Your personal data are handled and stored in accordance with our Data Protection Policy
Dr Anke Hensiek needs to gather information about individuals in order to provide best medical care. This data will include patient identifiable information such as names, date of birth, postal addresses, email addresses, telephone numbers or other information relating to individuals’ data protection risks. Any personal data is collected, handled and stored to comply with the Data Protection Act. Our data protection policy applies to all staff working for and on behalf of Dr Anke Hensiek. Dr Hensiek is registered with the ICO and staff covered by this policy have received data protection training.
We store medical records as paper records or electronic files stored on computers. We may also keep records of medico-legal instruction, medico legal patient records or other medicolegal documents which are usually kept as files stored on Computer.
The only people able to access these data are those who need it for their work. When access to confidential information is required it must be requested. Personal data is not disclosed to unauthorised people. Data is regularly reviewed and updated if it is found to be out of date. If no longer required, it will be deleted and disposed of.
When data is stored on paper, it is kept in a secure place where unauthorised people cannot see it. When not required, the paper or files are kept in a locked drawer or filing cabinet. Data printouts are shredded and disposed of securely when no longer required. When data is stored electronically it is protected by strong passwords that are changed regularly and never shared. All servers and computers containing data are protected by approved security software and a firewall.
We will not disclose your information to any other third parties except where there is a legitimate interest, for example, we may share information with your GP or other specialists involved in your care.
All individuals who are the subject of personal data held by Dr Anke Hensiek are entitled to be informed what information is held about them and why, as well as making a request to gain access to it. If an individual contacts Dr Anke Hensiek, we will aim to provide the relevant data within 14 days.
